The Rehabilitation Pavilion at The Weils, part of a Montefiore Senior Community in Chagrin Falls, has been named to the 2012 Honor Roll in U.S. News & World Report’s annual Best Nursing Homes.

The U.S. News Honor Roll highlighted 39 nursing homes with perfect performance ratings over the past year.Out of more than 15,500 facilities that U.S. News reviewed, the Rehabilitation Pavilion at The Weils is the only Ohio facility to receive four straight quarters of perfect five-star ratings from the federal Centers for Medicare and Medicaid Services – the government agency that sets and enforces standards for nursing homes (defined as facilities or portions of facilities enrolled in Medicare or Medicaid that provide 24-hour nursing care and other medical services) in all three areas – health inspections, nurse staffing and quality of care.

The new 29-bed, rehabilitation pavilion on The Weils campus offers a soothing and healing environment for short-term post-hospital nursing and rehab care. Our rehab therapists focus on helping individuals reach their goals and maximize their independence and strength.

Rehab 2The Weils offers innovative rehabilitation modalities to help bridge the gap between rehab and community reintegration. Skilled nursing care combined with physical, occupational and speech therapies after an illness, injury or surgery, helps get you stronger and prepares you to transition home and being more independent.

Our interdisciplinary clinical team, led by a medical director, includes physicians, rehab therapists, nurses, dietitians, social workers, activity coordinators and nursing assistants. They work together, along with the patient and family, to ensure the best treatment options and outcomes based on individual needs.

SuitePrivate and semi-private comfortable, bright rooms with warm lighting, matching furniture and laminate wood flooring contribute to a restful yet productive stay. Private bathrooms with safety assist equipment complete your rehab experience.
